Referring patients is quick and easy.
How it Works:
Submit a Referral Form online , via PDF or fax (661) 424-7347
Once we receive your complete referral form, we will contact you within 15 minutes
We will let you know within the first 5 minutes of the call if we can accept your patient. (We know how busy you are!)
We are able to accept and pick up patients from your hospital 24 hours a day
We are always available to talk patients to answer any questions and help ease the transition

Things to know:
We do not require patients to come with their medications filled. We can assist them with filling any prescriptions
Our facilities are designed to accommodate wheelchair-bound, obese and other handicap patients
We provide patients with transportation to all follow-up visits
We help set up Home Health, Dialysis, and Methadone
